Introduction

Hello!  My name is Chloe’ Koval and I have been the school nurse at Shanksville-Stonycreek School District since 2020.  I graduated from Shanksville in 2011 and received my Bachelor’s degree in Nursing from UPJ in 2015. I completed my School Nurse Certification from Eastern University in 2019. I am BLS, ACLS, and PALS certified.  Prior to becoming a school nurse, I worked at UPMC Somerset in the ICU for 5 years and I still work there when I am able to.  Outside of school, I enjoy spending time with my family as much as I can.  My husband, Dylan, and I have two kids, Miles and McKinley.

I am so excited to help care for your kids during their school day and to be a part of this school family. Below you will find different forms that your child may need for the school year and additional information and resources.  Please feel free to contact me with any questions or concerns you may have. These past few years have been nothing short of challenging but I’m excited to see what the coming years have to offer (hopefully some normalcy).

Reminders

Please see the Information and Forms section for any forms needed and additional information.

  • Please keep your child home if they have a fever (100.4 or higher), are vomiting, having diarrhea, excessively coughing, or have an eye infection.
  • Up-to-date immunization records are due by the first day of the new school year (PreK, Kindergarten, 7th, and 12th).  If records are not complete or submitted by day 5 of the school year, the student will be exempt from school until immunizations are up-to-date.
  • For students in grades that require a dental or physical exam, please remember to submit the exam prior to the end of the school year.
  • If your child has a life-threatening allergy, asthma, or serious medical condition that requires intervention (i.e. seizures), please have their doctor complete an action plan and submit it to the school nurse.
  • Students are not permitted to carry medication with them at school (exceptions: inhalers and EpiPens with physician note). If a student needs to have medication during school hours, the medication must be brought in by an adult with appropriate paperwork.

 

Mandated School Health Screenings
  • Height/Weight: Entry-12th Grade
  • Vision: Entry-12th Grade
  • Hearing: Entry-3rd Grade, 7th and 11th Grade
  • Scoliosis: 6th and 7th Grade
  • Physical: Entry, 6th and 11th Grade
  • Dental: Entry, 3rd and 7th Grade

**Referrals for vision, hearing, or scoliosis will be sent home as needed. A full health screening report will be sent home by the end of the school year.
