All students must submit the Medical Treatment Consent Form in order to receive any of our standing order medications.  The form also includes emergency contact information and general medical history to help treat the student accordingly. 

MEDICAL TREATMENT CONSENT FORM

Per the PA Department of Health, a physical exam must be completed for entry into school, 6th grade, and 11th grade. A sports physical can be accepted in place of a general physical exam.  Your child's family doctor can best evaluate your child's health and assist you in obtaining any necessary evaluation and treatment. However, our school physician will perform a physical exam if your prefer.  There is no cost to you for the exam. The school exams are typically completed on the last day of school after dismissal. 

If you are having a sports physical completed by our school physician, the cost is $18.  Checks can be made out to Somerset Health Services and are due at the time of the exam.

Per the PA Department of Health, a dental exam must be completed for entry into school, 3rd grade, and 7th grade. Your child's family dentist will best be able to evaluate your child's need and assist you in obtaining necessary treatment or corrections. Our school dentist can perform basic exams if you prefer. The school dental exams are typically completed some time in May.

DENTAL EXAM FORM

Per the PA Department of Health, up-to-date immunization records are due by the first day of the new school year (PreK, Kindergarten, 7^th, and 12^th) unless the student has a medical or religious exemption.  If records are not complete or submitted by day 5 of the school year, the student will be exempt from school until immunizations are up-to-date.  Please see additional information in the forms below explaining revisions, including guidelines for provisional enrollment and exclusion from school.

IMMUNIZATION INFORMATION

All students for entry into school

• 4 doses of DTaP (1 dose on or after 4^th birthday)
• 4 doses of Polio (1 dose on or after 4^th birthday)
• 3 doses of Hepatitis B
• 2 doses of MMR
• 2 doses of Varicella or evidence of immunity

Entry into 7^th Grade

• 1 dose of TDaP
• 1^st dose of MCV

Entry into 12^th Grade

• 2^nd dose of MCV

If your child has a life-threatening allergy, asthma, or serious medical condition that requires intervention (i.e. seizures), please have their doctor complete an emergency action plan (EAP) and submit it to the school nurse.

Please keep your child home if they have a fever (100.4 or higher), are vomiting, having diarrhea, excessively coughing, or have an eye infection.  For all illnesses, students/staff must be fever free and no longer vomiting/diarrhea for 24 hours without the use of medication prior to returning to school.

If Covid-19 positive, the CDC guidelines are as follows…

• Isolate at home for 5 days from start of symptoms (symptom start is day 0)
• If symptoms are improving and fever free for 24 hours, return wearing a mask for days 6-10.
• If symptoms continue or you are unable to wear a mask, isolate at home for the full 10 days.

We strongly encourage students and staff to follow the recommended guidelines provided by the CDC. Student absences due to Covid-19 positive cases will be excused.  If you choose to report a Covid-19 positive case or would like to discuss isolation options, please call 814-267-4649, ext. 219.

The form below is required if a student needs a prescription medication during school hours.  It must be completed by their doctor and have a parent's signature as well.

MEDICATION CONSENT

Our school is very fortunate to be able to offer the ImPACT test to all students in grade 6th-11th, not only to those who are participating in sports.  ImPACT is a nationally renowned concussion screening program.  The computerized screening is a sophisticated research-based software tool developed to help sports medicine clinicians evaluate recovery following a  concussion.  The ImPACT program evaluates and documents multiple aspects of neurocognitive functioning including memory, brain processing speed, reaction time, and post-concussive symptoms.  The data recorded during the initial screening will be used as a baseline to help the medical staff in determining treatment options and a safe return to normal activity following a head injury or concussion. 

We perform the ImPACT test in the spring on all students in grades 6th, 8th, and 10th.  It is required for students who are participating in sports.  It is optional for students who will not be participating in sports but please consider this opportunity as an added benefit to the overall care and well-being of your child at no cost.  Letters to parents will be sent home 1-2 weeks prior to the date of the test. 

**If your child experiences a head injury in or outside of school, please let me know so I can perform a quick assessment on them.  If it is necessary, we can perform a post-injury ImPACT test.  A copy of the results will be sent home for you to utilize with the students doctor.
